Policies 

 

I have not withheld any information regarding my health and the information I have given is true to the best of my knowledge.  

I consent to before and after photos and allow these to be posted publicly for Lightning Lash Studio's advertising.  

I understand that if I do not attend my patch test then my treatment may be cancelled.  

I comply with the strict no show policy and understand that if I cancel my appointment within the 48 hours before then I will be charged 50% and if I no show to my appointment I will be charged 100%. 

I understand that I will need to pay 50% deposit for my treatment that is non refundable and only transferrable up to 48 hours before the treatment. 

 I understand that I will need to pay for my treatment at the time of my appointment.  

I understand that if I am late to my appointment more than 15 minutes that I will be marked as a no show and that I will have to reschedule. 

I understand that I must come alone to my appointment and children are not permitted in the salon. 

I understand that if I am unhappy with anything I will notify Lightning Lash Studio within the 48 hour time frame after treatment with picture evidence, and a fix or removal will take place.  

I understand that once the treatment is completed that I will not be able to get a refund. 

 I understand that if I have gone somewhere else to have an issue rectified then I cannot hold Lightning Lash Studio responsible.

I have been offered a patch test and understand that it is compulsory a minimum of 48 hours before the treatment. 

I accept there may be a risk of an adverse reaction to this treatment, including the possibility of swelling, irritation and redness.  

I understand an adverse reaction can happen at any time and not necessarily after the initial treatment.  

I agree to follow all aftercare instructions given to me and stated on the Lightning Lash Studios instagram page, if I have not followed these then I know that I cannot hold Lightning Lash Studio responsible.  

I understand that if I have any issues with my lashes I will notify the lash technician with in the first 48 hours of the treatment and send picture evidence.  

I understand that if any of these reactions occur I should contact my lash technician immediately who may request I come back in for a removal procedure and I shall not remove the lashes myself. If symptoms persist I must seek medical attention.  

I understand that in the case of a reaction, I will not hold my lash technician responsible in anyway, this includes any of the reactions specified above and/or any other reaction I may have.   

I understand that a full set of lash extensions can make the appearance of my own lashes about 30-50% thicker, and make my lashes appear 20-50% longer.  

I understand that this is a semi-permanent procedure, as my natural lashes will continue to grow and fall out normally, making touch-up appointments necessary to maintain the original look achieved by replacing the lashes that have fallen out. Most clients require a appointment every 2-3 weeks dependant on how good the aftercare is. 

I understand that while every attempt will be made to provide me with the length and fullness I have chosen, my final result may not be what I initially envisioned. 

 I understand that additional conditions could occur or be discovered during the procedure which could affect my ability to tolerate the procedure. 

I understand that if I have any concerns, I will address these with my lash extension specialist. 

I give permission to my lash extension specialist to perform the lash extension procedure we have discussed, and will hold her and her staff harmless and nameless from any liability that may result from this treatment. 

I have accurately answered the questions above, including all known allergies, prescription drugs, or products I am currently ingesting or using topically I understand my lash extension specialist will take every precaution to minimize or eliminate negative reactions as much as possible. 

In the event I may have additional questions or concerns regarding my treatment, I will consult the lash extension specialist immediately. 

I certify that I have read, and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. 

I understand the procedure and accept the risks. 

I do not hold the lash extension specialist, whose signature appears below, responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed today.

I agree that I have answered all medical questions to my best knowledge and have disclosed anything that could affect me going forward with the treatment including if anything medically changes within multiple sessions.

 I agree to come to my appointment following the before application points. 

I understand that if I don’t want to wear the disposable knicker provided then I will need to bring my own that could possibly get stained.  

I understand that after my appointment I will need to bring dark loose clothing including flip flops to wear after my treatment.

I understand all the aftercare instructions provided on Lightning Lash Studio’s page I understand all skin is different so I may not get the same results as someone else. 

I understand that there is a small percentage of people whose skin may not react favourably to spray tanning. For this reason, we do NOT advise being sprayed for the first time when your appearance is critical; (wedding/special occasion/prom) Please schedule a practice session 30 days before your event, for best results.  

I understand that if I am pregnant I will need to consult a doctor first

 I understand that the product does not contain a sunscreen therefore I am not protected against sunburn. 

I understand that the results of my spray tan are directly affected by my adherence to aftercare instructions

 I understand that I will asked what look I want for my tan during the consultation and this will be communicated to me on whether it matches Lightning Lash Studios recommendation.  

I take sole responsibility of any reaction I may have, staining of clothing and/ or personal belongings.

I understand that when booking an infill I am required to have 50% of my lashes left, my tech will guide me on the timing of infill I require

I understand that once my treatment has been booked I cannot change the treatment within the 48 hour window.
